Analysis was performed using a qualitative framework approach. RESULTS: Participants reported a variety of EPIOC uses, including shopping and some social contact. Moderate satisfaction with the chair was reported. Use was compromised by indoor and outdoor environmental barriers; and the chair not meeting users’ needs. Accidents were rare, but many users still experienced insecurity in the chair. High levels of satisfaction with the service were reported, although concern was expressed over length of waiting times. CONCLUSIONS: EPIOCs proved useful to most older people with disabilities. However, even those who were satisfied reported only moderate use of the chair outdoors. Limited use related to an infrequent need for outings, outdoor barriers, feelings of insecurity over chair safety and lengthy waiting times for chair delivery and required modifications. Service providers should be aware that older EPIOC users may require extra support and the provision of timely adjustments to increase chair use.

INTERVENTIONS: All subjects followed the regular rehabilitation program. MAIN OUTCOME MEASURES: Peak oxygen uptake (Vo(2)peak) and peak power output (POpeak) determined in a handrim medical scooters peak exercise test, peak muscle strength of the upper extremities, and pulmonary function. RESULTS: A significantly larger increment in Vo(2)peak, POpeak, and elbow extension strength was found in subjects with paraplegia during clinical rehabilitation. No such effect was found in subjects with tetraplegia. In the postrehabilitation period, no influence of hand cycling on any outcome measure was found in subjects with paraplegia or subjects with tetraplegia. CONCLUSIONS: After correction for baseline values and confounders, regular hand cycling (once a week or more) appeared to be beneficial for improving aerobic physical capacity in persons with paraplegia during clinical rehabilitation. The small and heterogeneous study groups may have hampered the finding of positive results of hand cycling in persons with tetraplegia.

Validity was determined by comparing 6-MAT results with peak oxygen consumption (VO2peak). SETTING: Tertiary rehabilitation center. PARTICIPANTS: Thirty subjects with SCI (mean age, 36.3y; 83% male). INTERVENTIONS: Not applicable. MAIN OUTCOME MEASURES: Subjects were evaluated on the 6-MAT and handicap scooters a VO2peak test. RESULTS: All subjects were able to complete the 6-MAT. Test-retest reliability of steady-state oxygen consumption (VO2) and heart rate during the 6-MAT were excellent (intraclass correlation coefficient ICC, .81; 95% confidence interval CI, .58-.92; ICC=.90; 95% CI, .75-.96, respectively). The correlation between VO2peak and 6-MAT VO2 was excellent (r=.92) and the correlations between VO2peak and 6-MAT heart rate (r=.63) and VO2peak and 6-MAT medicare power wheelchairs output (r=.73) were good. CONCLUSIONS: This study showed that the 6-MAT has acceptable values for test-retest reliability and validity. The 6-MAT should be further tested for responsiveness to enhance its use as a clinical tool.

The experience of senior stroke survivors: factors in community participation among wheelchair users
PURPOSE: This qualitative study investigated the lived experience of senior stroke survivors who used prescribed medicare power wheelchairs in their home and community. METHODS: The study involved semi-structured, in-depth interviews that were conducted with 10 participants, aged 70 to 80, who had used a power wheelchairs for a mean of 5.6 years. All participants used manual wheelchairs and two additionally used a power wheelchair and a scooter. RESULTS: A constant comparative inductive method of analysis of the data yielded several themes which are examined within the International Classification of Functioning, Disability and Health framework of body functions and structure, activity limitations, participation, and contextual factors. Overall, the wheelchair, manual or power, was an enabler of community participation among the participants. However, the wheelchair’s use also created problems, such as limiting destinations and creating increased dependence on others. PRACTICE IMPLICATIONS: A balance scale model was proposed to facilitate an understanding of how the factors affecting community participation interact in this population. This model can assist occupational therapists who prescribe wheelchairs for stroke clients for use in the community.

Although all the ramps were designed to load powered wheelchairs into motor vehicles, they were manufactured in different designs. The ramps were based on a “singlewide” platform or “channel” design. Some ramps had fixed dimensions, whereas others could be reduced in size because they were telescopic or designed to allow folding. Overall, the ramps could be divided into four subgroups on the basis of their key features. These were horizontally and longitudinally folding ramps, telescopic ramps, and ramps with fixed dimensions. The telescopic ramps could be subdivided into “U”-shaped gutter ramps and reverse profile ramps. Product appraisals and trials involving wheelchair users and caregivers of wheelchair users were done to evaluate each of these ramp designs. Although wheelchair ramps are available in a wide range of designs and configurations, we found that no single ramp design successfully met the needs of all wheelchair users or their caregivers. The evaluation highlighted a number of specific problems and potential hazards. Some ramps were found to move during a maneuver, showed poor stability when used with some vehicles, or were too narrow to allow wheelchair castors to pass through the channel without jamming. Some features, such as handles and locking mechanisms, influenced the ease with which the caregivers could use the ramps. Wheelchair users preferred the wide platform ramps because they were able to drive up these with ease and little preparation. The caregivers preferred folding or telescopic channel ramps because these were easier to handle and store.
